Job description
- Conduct home visit and assessment of client’s needs and financial situation
- Prepare social report and present to Senior Social Worker for approval
- Develop care plans and enroll clients for elderly service program
- Provide practical support to operational needs of department
- Prepare, file and maintain proper documentation and updates of clients’ data
- Regular follow up and review to assess progress and additional needs of clients
- Engage and coordinate with different stakeholders to support delivery of care plans for clients
Requirements
- Diploma holder in any discipline
- Experience in social service sector will be an advantage
- Computer literacy in Microsoft office
- Able to speak English, Mandarin and dialects
- Good writing, communication, and interpersonal skills
- Team player with integrity, patience and commitment
Job Description
- Delivery meals to frail home bound elderly clients
- Accompany and assist elderly for medical appointments
Requirements

- Able to work on weekends (rotational basis)
- Able to work on public holidays
- Possess Class 3 driving license and able to drive a van
Job Description
- Assist Chief Cook to plan menus & procure groceries and kitchen equipment
- Assist Chief Cook to prepare and cook meals for elderly, children and staff
- Clean kitchen equipment and work area
- Workdays: Monday to Saturday

- Possess professional cooking certificate
